HPV Vaccination Reminder
Dear parents/carers,
This is a reminder for the Parents of Year 8 students who have not yet returned their HPV immunisation forms.
These immunisations will take place on 7th January 2022 when we return after the Christmas break. In order to allow the nurses time to order the vaccine please can you return your form by 10th December at the latest.
The form should be returned showing whether you do or do not consent and be signed by a parent/carer.
